I think that video might have been staged, but it gives the general gist of an electrical short circuit when dealing with heavy duty switchgear.
The breaker (big switch) he was racking in (using a handle to turn a threaded bar below the switch to drive it in to make contact with the connections) looks like it had a short on it.
This can easily happen if the maintenance crew who maintained the breaker had left a spanner or other tool inside it when they finished.
We have to wear nomex overalls, cotton under wear

, flash hood, flash guantlets, ear protection and a hard hat with a face shield for potection when racking breakers in and out of live boards
